The Waterloo Wellington CCAC is here to help you find your way through Ontario’s health care system by giving you information & advice about the options available in our community.  
 
Our services help people avoid hospitalization, or return home quicker after being in hospital to recuperate in the comfort of their own homes. When living at home is no longer possible, we will help with admission into long-term care. 
 
Our staff develops an individualized care plan & co-ordinate services such as nursing, physiotherapy, occupational therapy & personal support for you when you are recovering from illness or injury, & for those who are living with chronic illness or disability. We work in partnership with clients, families, providers, hospitals, long-term care centres & others to ensure that care is available at the right time & place. 
 
Our services are provided at no cost & are 100% publicly funded. Anyone can call: you, a caregiver, a family member, friend, or physician.
