At the Grand River Community Health Centre, we believe in a positive, proactive approach to health and wellbeing. Our services are client-centred and community based, with a focus on those who face barriers to access.

Our professional staff work as a team to support each client's journey to improved health & wellness. Our services include:

Community Health Centres focus on keeping people well - not just treating them when they are sick. To do this, CHCs deliver primary care services in combination with health promotion and illness prevention services. And what makes CHCs stand out is a strong community development focus. So in addition to promoting the health of individuals and families, CHCs create and support initiatives that address social, economic and environmental issues impacting people's health.
