We support patients and their families by raising money for special projects that benefit palliative care. With expert advice from the Champlain Hospice Palliative Care Program, OutCare selects high-priority projects with a regional impact so that regardless of which organization families turn to, they receive the skilled and compassionate help they need and deserve.

Over the years, our activities have resulted in $1.5 million in support for programs and services throughout our community. Whether helping to make more care available, supporting caregivers, or ensuring proper training is offered, OutCare donors are making a difference.

La Fondation OutCare est un organisme de bienfaisance qui aide les patients en soins palliatifs et leurs familles en recueillant des fonds pour offrir plus de soins et d’appui lors de ces moments difficiles. Grâce aux conseils des experts du Programme de soins palliatifs Champlain, OutCare identifie les projets qui contribuent à améliorer les services et les soins disponibles,  afin que peu importe l’organisme auquel les familles demandent de l’aide, elles reçoivent les soins de qualité qu’elles méritent et dont elles ont besoin. 

Nos quatre priorités sont : augmenter l’accessibilité des soins pour qu’ils soient facilement disponibles lorsque requis; supporter les membres essentiels de l'équipe de soins palliatifs que sont les familles, les aidants et les bénévoles; améliorer la diffusion de l’information et accroître la connaissance par l’éducation.
