Background 

Flemingdon Health Centre is a non-profit registered charity that has been operating in Toronto since 1975. We provide health and wellness programs and services to the neighbourhoods we serve. These programs and services include: 

• Primary Health Care: including access and attachment to physicians and nurse practitioners  

• Individual and Family Counselling: including one-on-one services and group programs with our social workers and mental health workers 

• Chronic Disease Prevention and Management: including a focus on diabetes, lung health and heart health with access to dietitians, nurses and chiropodists 

• Health Promotion programs and initiatives for seniors, women, youth and newcomers including exercise, wellness and healthy cooking 

FHC also leads a number of neighbourhood initiatives and supports grassroots work.
