Bridges Community Health Centre serves the communities of Fort Erie, Port Colborne and Wainfleet with a focus on the needs of seniors, children and youth, newcomers to Canada, and families or individuals with low incomes.
The staff at Bridges CHC includes physicians, nurse practitioners, nurses, dietitians, diabetes educators, social workers, health promoters, community health workers, a physiotherapist, kinesiologist and physiotherapist assistant.
Our primary care team, support clients of Bridges CHC and our health promotion programs are available at no cost and open to all members of the community who are looking to improve their health. To promote healthy living in our communities, our programs and peer support groups can help you with smoking cessation, diabetes education, mental health, chronic pain management, stress management, exercise and healthy eating.

Community Health Centres deliver primary care services in combination with health promotion and illness prevention services with a strong community development focus. In addition to promoting the health of individuals and families, CHCs are involved with initiatives that address social, economic and environmental problems negatively impacting people’s health. Governed by community members, and working hand in hand with the people they serve, CHCs also prioritize improving the health and wellbeing of populations who have traditionally faced barriers accessing health services.
