Wellfort Community Health Services is a vibrant Community Health Centre providing primary health care, focusing on health promotion and prevention to the residents of Bramalea and Malton since 2008. 

We are an interdisciplinary team of health professionals: physicians, nurse practitioners, nurses, chiropodists, dietitians, physiotherapists, social workers and dentists who collaborate to provide a holistic approach to health care. We address both health and social needs. We value diversity and provide culturally competent care.

We do this with a family of 5 targeted programs operating in Brampton and Mississauga:

Bloom Clinic provides testing, treatment and support to people living with, affected by, or at risk of HIV and Hepatitis.

DEP (Diabetes Education Program) provides free culturally-sensitive diabetes education, management and support for diabetes, in addition to individual and/or group sessions for people that are pre-diabetic or those with Type 2 diabetes. 

Health n'Smiles provides a variety of dental services focusing on improving oral health through prevention, education and quality dentistry. We deliver the Healthy Smiles Ontario Program for children aged 0 - 17, and the Region of Peel Seniors' Dental program, for seniors over the age of 65.

Four Corners Health Centre is the Malton Satellite of Wellfort. Here we  provide primary care services and run programs to address the needs of our clients, including chronic disease self-management, understanding hypertension, the francophone program Aging at Home, and WellCare, giving support to caregivers and family members of people living with Alzheimer and Dementia. 

Bramalea Community Health Centre provides primary care in Brampton, and continues to strive to provide the community with access to comprehensive health care services for our priority populations: non-insured, refugees, newcomers, low-income, S2LGBTQ+, and trans-identified people. Pre-natal services have been expanded by adding a Mid-Wife to the team, as well as weekly Teen Prenatal groups.
