Established in 1988, Community Care Northumberland strives to be the best, multi-service, volunteer-based community support organization in Northumberland County. Our over 900 regional volunteers enable clients to experience a higher quality of life by strengthening their connections with the community.  They along with our professional staff accomplish this through the delivery of the following essential community support services:

  • Community Diners - Meals in a social enviroment
  • Friendly Visiting - Compansionship individually or in a group session

  • Home at Last - Transition service from hospital to home

  • Home Help & Maintenance Programs - Assistance with routine indoor and outdoor household tasks

  • Hospice Palliative Care Programs - Visiting hospice programs open to indidividuals of all ages living with a life-threatening illness or in need of end of life care. 

  • Meals on Wheels Programs - Hot or frozen meals delivered to the client's door

  • Supports for Caregivers - Assistance with caregiving responsibilities

  • Telephone Security Checks - Service for isolated persons to receive a reassurance call

  • Transportation Services including Accessible Transportation Options - Volunteer, rural and accessible services that assist clients to travel to their medical, social and recreational appointments.

  • Wellness, Social and Recreational Programs -  Workshops, fitness, hobby-based activities

These programs operate throughout Northumberland County from Community Care Northumberland's six local offices located in - Brighton, Campbellford, Cobourg, Colborne, Hastings and Port Hope, Ontario.  Their support services to seniors and adults with disabilities allow them to remain independent within their own homes and communities. Their visiting hospice palliative care programs is open to individuals of all ages living with life-threatening illness or in need of end of life care, with on-going support to their caregivers/friends and families.  Clients can access services directly or be referred through family, friends or other community agencies.
