Rapids Family Health Team, located in Sarnia Ontario serves over 70,000 patients and offers many prevention programs for all ages. We offer a Chefs in Training program that includes 4 different programs designed specifically to target different age groups from ages 2 years to 18 years.  We are currently also providing programs to our local aboriginal population and are in the process of developing a French program for grades 7 and 8. We also have many emotional/mental health programs, including suicide prevention and dealing with loss of a loved one by suicide, chronic disease prevention including Diabetes, COPD and Balance and Bone Strength Programs.
