Our Mission
Street Health works to improve the health and well-being of individuals who are homeless and under-housed in southwest Toronto by addressing the social determinates of health through services, advocacy and education.
About Street Health
Street Health provides healthcare where people are at. Services are offered at Street Health (Dundas and Sherbourne Streets) and at community partner locations (homeless shelters, drop-ins and on the street). All services are client centered and focus on a harm reduction approach.
The people we work with have lives characterized by extreme poverty, chronic unemployment, insecurity in housing, poor nutrition, high stress and loneliness; they also have more frequent and serious illnesses, and die younger on average than the general population. Our services include outreach nursing, mental health support and case management, secure mail service, identification replacement, harm reduction and client services which offer personal care/hygiene, information and referrals.
